Enrollment Forms
The deadline for submitting ALL applications for the 2008-2009 enrollment period is
February 13, 2009 (U.S. Post Office postmark or hand-delivered, NOT e-mailed). If mailing, send to: The Benjamin Franklin Classical Charter Public School, 201 Main Street, Franklin, MA 02038.
There are three different application forms, depending on the status of the family:

Sibling application:
To be used only for brothers and sisters of students currently enrolled at the school. This application form is for all grade levels, kindergarten through grade 8.
NOTE: Siblings still on the wait list for the current year,
2008-2009, and who reapply for admission, remain on the wait list for next year’s classes
(2009-2010) in the same order as their positions for 2008-2009. Please fill out an application form for these siblings already on our wait list in order to confirm your interest in remaining on the wait list.

Resident application:
All Franklin-resident families new to the school use this form for students entering kindergarten through grade 8, as of September,
2009. Use one form per student and provide all of the information requested on the form.

All non-Franklin-resident families new to the school use this form for students entering kindergarten through grade 8, as of September,
2009. Use one form per student and provide all of the information requested on the form.

We are not able to accept these forms via the internet. Therefore, you may print the forms from our website, but you must submit the hardcopy form by the deadline as stated above.
